General Surgery
General surgery is a discipline that requires knowledge of and responsibility for the preoperative, operative, and postoperative management of patients with a broad spectrum of diseases, including those which may require nonoperative, elective, or emergency surgical treatment. The breadth and depth of this knowledge may vary by disease category. Surgical management requires skill in complex decision making and general surgeons should be competent in diagnosis as well as treatment and management of the disease process including operative intervention.

1- Please provide us with a brief statement regarding:

3- Please provide us with any available reports from your treating physician that include:
⁃ Brief medical / surgical history

⁃ Lab results from blood tests such as CBC, CMP.
⁃ Radiographic data such as X-rays, CT scan, MRI.
5- Please provide us, if available, with reports of:
– Pertinent lab tests such as Lipase, Thyroid function, Tumor Markers
– Nuclear scans or Endoscopy results
– Biopsy results
